The Colorado Coalition for the Homeless Stout Street Clinic

Colorado Coalition for the Homeless logo pic Jessie’s Homes for Families is a Colorado-based organization dedicated to ending homelessness. Through a partnership with the Colorado Coalition for the Homeless, the organization plans to construct Renaissance at North Colorado Station, which will offer 129 transitional housing units for families in need.

Renaissance at North Colorado Station will connect families with a wide range of services that they need to transition into permanent housing, including shuttle access to Stout Street Health Center. Funded by the Colorado Coalition for the Homeless, the clinic offers services to individuals free of charge.

At Stout Street Health Center, individuals of all ages will gain access to a skilled multidisciplinary team that understands the unique mental and physical health issues often associated with homelessness. The team places patients at the center of their health care and offers the information and advice necessary for them to make the best decisions for their own circumstances.

In addition to health care professionals, patients have access to case managers, consumer peer mentors, and outreach workers.
